Additional Items Required by States for Nursing Home MDS 3.0 Assessments and State Section S Items for October 1, 2023 Implementation

June 9, 2023 … States can require additional CMS defined items on their Quarterly (NQ) or PPS (NP) assessments. Please note: This is a change to the items on the state’s OBRA quarterly (NQ) and/or PPS (NP) records. It is not a different type of comprehensive (NC) assessment. These items must be sent in on the NQ or NP assessments. If providers send in an NC that is missing any comprehensive items, such as the Section V items, it will be rejected. CMS does not create state-specific printable item subsets.

Note: CMS has removed items A1500, A1510A-C, and A1550A-E, Z from the NQ and NP item sets as of October 1, 2014 so they are longer collected on NQ/NP records with a target date on or after October 1, 2014. These items are CMS required items on the MDS 3.0 NQ and NP assessments with a target date prior to 10/01/2014 as well as the comprehensive assessments. As of 10/01/2014, CMS has removed these items from the NQ and NP assessments. Due to the skip pattern on the items, states cannot add these as additional required items on their NQ/NP assessments.

These items are: S0200A, S0200B, S6060A, S6060B, S6060C, S6300, S6301, S6302, S6303A, S6303B, S6303C, S6303D, S6304A, S6304B, S6304C, S6304D, S6304E, S6304F, S6304G, S6304H, S6304I, S6305A, S6305B, S7001, S7002, S7500A, S7500B, S7500C, S7500D, S7500E, S7500F, S7500G, S7500H.
