Home Health Agency (HHA) Review and Correct Report Enhancements

An updated version of the HHA Review and Correct report is now available in iQIES.  This version includes the following updates:

1.  Removal of the following quality measures and associated data:

  • Improvement in Pain Interfering with Activity
  • Improvement in Status of Surgical Wounds
  • Depression Assessment Conducted
  • Multifactor Fall Risk Assessment Conducted for All Patients who Can Ambulate
  • Diabetic Foot Care and Patient / Caregiver Education Implemented during All Episodes of Care
  • Pneumococcal Polysaccharide Vaccine Ever Received

The above measures and data will no longer display on the HHA Review and Correct report if the End Date Range selected is beyond Q2, 2020. These same measures were removed from the HHA Process Measure and Outcomes reports in February 2020 and are now being removed from the HHA Review and Correct report as data for these measures will no longer be reported publicly after the October 2020 Home Health Compare refresh.

2.  Updated two measure names as identified below:

  • Changed “Application of Percent of Residents Experiencing One or More Falls with Major Injury (NQF #0674)” to “Percent of Patients Experiencing One or More Falls with Major Injury”
  • Changed “Application of Percent of Long Term Care Hospital Patients with an Admission and Discharge Functional Assessment and a Care Plan That Addresses Function (NQF #2631)” to “Percent of Patients with an Admission and Discharge Functional Assessment and a Care Plan That Addresses Function

 3.  Removed the NQF numbers from the following measure descriptions:

  • Improvement in Ambulation/Locomotion
  • Improvement in Bathing
  • Improvement in Bed Transferring
  • Improvement in Management of Oral Medications
  • Timely Initiation of Care
  • Improvement in Pain Interfering with Activity
  • Improvement in Status of Surgical Wound
  • Depression Assessment Conducted
  • Multifactor Fall Risk Assessment Conducted for All Patients who Can Ambulate
  • Diabetic Foot Care and Patient Education Implemented during All Episodes of Care

 4.  Updated the Report Version Number to 3.0

 5.  Updated the patient-level results data display in the in the Comma Separated Value (CSV) version of the report as follows:

  • Quality Measure (QM) order in the file was updated to match the QM order in the PDF version of the report.
  • Reporting Quarters now display in descending order for each measure
  • Data are sorted in alphabetic order by patient last name and then patient first name
